Nursing Home Social Worker
Holliswood Center is seeking a Social Worker for our Skilled Nursing Facility in Queens, NY. The ideal candidate will have experience in a long-term care / sub-acute setting and must have previous healthcare experience.
Must be trained in Point Click and savvy using HER system
DUTIES:
- Maintaining and/or improving the residents ability to control everyday physical needs and mental/psychosocial needs
- Identifying individual social and emotional needs
- Developing and Maintaining individualized care plans
- Participating in the planning of the residents admission, community re-entry, or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
REQUIREMENTS:
- EXPERIENCE WITH MDS 3.0
- Bachelors Degree in Social Work or related fields.
- Proficient in discharge planning and memory support
- Long term care/sub-acute experience required
