Schenectady Center is actively seeking an experienced Social Worker for our Skilled Nursing Facility in Schenectady, NY. The ideal candidate will have prior experience in a long-term care setting, and experience in training and leading clinical staff.

DUTIES:

• Maintaining and/or improving the residents ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and Maintaining individualized care plans
• Participating in the planning of the residents admission, community re-entry, or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

REQUIREMENTS:

• EXPERIENCE WITH MDS 3.0
• Discharge planning experience required
• Long term care/ sub-acute experience required
• BSW/MSW is preferred
