Deptford Center for Rehabilitation and Healthcare is actively seeking a Social Worker for our Skilled Nursing Facility in Deptford, NJ.

The social worker will work with residents in the SNF/LTC facility by identifying their psychosocial, mental, and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs from admission to discharge.

Duties Include:

• Conduct psychosocial interview with each new admission
• Develop individualized care plans for each resident
• Invite families and residents to care plan meetings
• Participate in and document care plan meetings, family meetings, and discharge planning meetings
• Conduct scheduled BIMS/PHQ9 assessments, create supportive documentation, and complete MDS books as appropriate
• Document any relevant psychosocial information in residents charts, i.e. mood changes, family issues, resident to resident altercations
• Attend UR meetings
• Plan and execute safe discharges using Discharge Planning Checklist
• Receive complaints/grievances from residents and families, and report them to the Social Work Director
• Maintain current and accurate demographics and contact information on EHR and charts for all residents.
• Seek supervision and guidance from the Director of Social Work/designee

Requirements Include:

Candidates must have:

• CSW/BSW or LSW/MSW
• Long-Term Care / Subacute experience preferred
• Exceptional interpersonal & communication skills.
• Knowledge of applicable State & Federal guidelines.
