Occupational Therapist (Effective 2024-2025 SCHOOL YEAR)
Job Description
SUMMARY
Evaluates and develops an Individualized Educational Plan (IEP) to meet the specific needs of students who qualify for occupational therapy services.
MINIMUM REQUIREMENTS
Arizona Occupational Therapy License
Arizona IVP fingerprint clearance card
Verbal & Written communication skills in English and a demonstrated ability to read and comprehend written/graphic and oral instruction
Must release Social Security Number to be eligible for AHCCCS-Provider ID Number
ADDITIONAL REQUIREMENTS AFTER HIRE
Proof of immunity to rubeola (measles) and rubella (German measles), or proof of MMR immunization
Valid Driver's license (Arizona Drivers License required within 30 days of hire)
Reliable mode of personal transportation with evidence of insurance compliance greater than or equal to the state required minimum for car insurance as described under Governing Board Policy: EEB-R-1 Business and Personnel Transportation Services - Transportation by Employees.
MVReport - showing no more than the allowable points against Drivers License as described under Governing Board Policy: EEB-R-1 Business and Personnel Transportation Services - Transportation by Employees.

Location: Exceptional Education Department - District Wide - Sites May Vary
FTE: Multiple Positions - Hours Vary
Work Calendar: 9.5 month
Positions less than 30 hours per week are not eligible for district benefits.

Probationary Contracts will be issued for any contingent Certifications, Positions that are 0.6 FTE or less, and certified employees within their first three years with the District.
