Pay Range

$34. 56 - $44. 71/hr

Responsible for coordinating and implementing post-discharge plans in coordination with the Case Managers through theuse of Extended Care Information Network (ECIN). They are also responsible for assisting with advocacy and referrals toother community resources.

What You Bring/Job Requirements

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:

Knowledge of community resources used for discharge planning, hospital operations, excellentcommunication/presentation skills, knowledge of third party payment systems, Medicare/Medicaid programs.Maintains current knowledge base of community services through continuing education.Ability to multi-task, set priorities and maintain organization.Computer skills.

Graduate of Accredited Master's in Social Work Program

Current Colorado LCSW License

Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
