The Patient Care Coordination Manager has responsibility for the management of the Patient Care Coordination staff that work in partnership with physicians, facilities, discharge planners, social workers and clinic staff to promote coordination of care among patients between all settings including the Medical Surgical, OB, Senior Services, ER, hospital outpatient, and clinic settings across the Sioux Center Health footprint.

Managerial Responsibilities: Is responsible for the overall direction, coordination, and evaluation of this department. Carries out managerial responsibilities in accordance with the organizations policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Departmental Supervision: Social Services, Interpreter and Cultural Services

Education and/or Experience: RN required. Previous experience in care coordination preferred. Previous supervisory or management experience preferred. Bi-lingual experience preferred but not required.
