Job Description

Our skilled nursing facility is seeking an experienced and caring full time physical therapist to join our in-house therapy team. This individual would act as our residents' partner and advocate on their road to recovery, from diagnosis to the completion of rehabilitation. Resident quality of life, range of motion, and overall safety should be the sole focus of any candidate applying for this position. You will be expected to be through and communicative with residents and team members. The ideal candidate will be resident focused and able to work well with team members.

Job Summary

Resident evaluation and effective administration of treatment programs, actively participate in the interdisciplinary rehabilitation team, and contribute to community marketing activities. Ensures compliance with all state and federal regulations.

Performs resident evaluation & provides effective administration of treatment programs. Actively contribute to the interdisciplinary team. Assists residents to restore physical function by interpreting physical therapy evaluations and test results; determining physical therapy treatment goals and plans; administering manual exercises; instructing, encouraging, and assisting residents in performing physical activities and in using assistant and supportive devices.

ESSENTIAL FUNCTIONS

  • Review and evaluate physicians referral and residents medical record to determine Physical Therapy program required.
  • Meet with residents and evaluate each residents current level of functioning using approved evaluation techniques, tests and observations, and prepare written treatment programs based on evaluations.
  • Administer a prescribed Physical Therapy program to restore and maintain maximum function and prevent disability using a range of therapeutic modalities.
  • Coordinate physical therapy services with Occupational Therapy, Speech-Language Pathology and the Nursing department.
  • Maintain appropriate documentation in the resident chart, and re-evaluate resident progress and revise treatment programs when necessary.
  • Communicate Physical Therapy philosophy and treatment approaches and goals to residents, their families and other members of the team.
  • Supervise Physical Therapy Assistants and monitor productivity.
  • Develops physical therapy staff by providing information and developing and conducting in-service training programs.
  • Adhere to the facility mission & vision, policies and procedures.
  • Maintain productivity standards while prioritizing resident care.
  • Full utilization of technology provided to document and enhance client care.
  • Protects healthcare organization value by keeping information confidential; cautioning others regarding potential breaches.
  • Improves physical therapy job knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies.
  • Contributes to physical therapy and healthcare organization success by welcoming related, different, and new requests; helping others accomplish job results.
  • Any and all duties as assigned.


QUALIFICATIONS

  • Graduate of an accredited school for Physical Therapy and a valid registered license for the state in which employed.
  • Acute care, long-term care and/or rehabilitation experience of at least one year, preferred.
  • Ability to successfully teach others how to do something
  • Actively look for ways to help people.
  • Ability to communicate and adapt to different residents and personalities
  • Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
  • Able to work in a fast-paced environment, multi-task, prioritize, and meet deadlines.
  • Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
  • Effective interpersonal skills with strong focus on customer service.
  • Proficient use of computers and Microsoft Office: Word, Excel and Outlook.

PHYSICAL DEMANDS AND WORK ENVIRONMENT

The various responsibilities for a Physical Therapist are often physically demanding and require that therapists be in good physical condition.

  • Required to have close visual acuity to perform activities such as: preparing and analyzing data and figures, may include extensive reading and frequently operates a computer.
  • Frequently communicates with residents, staff, management, and clients. Must be able to professionally exchange accurate information in these situations.
  • Must be able to withstand prolonged or considerable walking, standing, balancing, reaching, pushing, pulling, lifting, stooping, bending, kneeling, or crouching.
  • Must be able to complete a max assist resident transfer (75% employee, 25% resident) independently.
  • Must be able to otherwise position or transfer residents as well as supplies and equipment.
  • Must have sufficient visual acuity and hearing to perform required therapy skills.
  • Heavy work, exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.

Work schedules vary at each location. You may be scheduled to work at any other location.
