Eastern Plumas Health Care is a small, non-profit, critical access hospital district, providing comprehensive medical services to Plumas County since 1971. We operate a 9 bed acute care hospital at our main Portola campus, which includes a 24 hour, physician staffed, emergency room and ambulance service.

In addition, three primary care medical clinics and a dental clinic offer residents a full spectrum of health care services in the Graeagle, Loyalton, and Portola communities. There are also hospital-based Skilled Nursing Facilities in Portola and Loyalton.

Surrounded by the Sierra Nevada Mountains with beautiful lakes and trails for hiking, biking, fishing and skiing, this facility is thriving and wants a highly motivated and experienced FNP or PA-C to provide excellent care to our community. Its an exciting opportunity to be an integral part of the successful growth this rural hospital is experiencing by providing expertise in our Portola Medical Clinic.

Position Summary:

We are actively seeking an experienced Physician's Assistant or Nurse Practitioner (PA/NP) to compliment our team of Primary Care Providers. This is a full time benefited position. The PA/NP assumes total care of clinic patients under the direction of a physician in the rural health clinic setting. Follows established standards, procedures, and practices for rural health clinics. Scheduling will vary with the needs of the Outpatient Clinics. Week days only, no weekends.

The FNP or PA-C will be responsible for direct patient care and for detailed adminstrative duties as designated by the Medical and Clinic Directors and in accordnace with State and Federal rules and regulations.

Requirements and Qualifications include:

  • Graduate from an accredited school of nursing and nurse practitioner program: OR
  • Graduate of an accredited program for Physician's Assistants, with Certification for practical application and practice of medicine.
  • Skilled in traditional disciplines of patient assessment and management.
  • 1-3 years experience as a practicing physician assistant.
  • Certified as a nurse practitioner by the State of California
  • Current CPR/BLS certification.

Principal Accountabilities:

Practice medicine using sound medical judgement and adhere to all ethical consideratons of the practice of medicine.

Follow all internal internal procedures and policies and be accountable for completion of clinical tasks

Develop and implement health plans for health promotion and maintenance. Provide periodic assessments, health education and counseling to patients and families.
