Prenatal Outreach Coordinator
Job Description

Provide a range of community and field-based prenatal outreach and care coordination to designated community sites in the community of Vista, Oceanside and San Marcos including outreach, health insurance education, and linkage to prenatal care. Assist in prenatal patient recruitment, prenatal program activities, and administrative support.
- Conduct community outreach, including community presentations, staffing at health fairs and events
- Create partnerships with local high schools, Rehab facilities and other community programs for recruitment of prenatal patients
- Create outreach plans to increase prenatal program & CPSP enrollment
- Contact community programs to present prenatal services available
- Provide enrollment education
- Instruct clients on the documents necessary for the eligibility process, and provide an overview of available healthcare options and services offered in prenatal program
- Refer clients to support services and programs - CBE, Lactation, CPSP
- Compile statistics and reports on the number of outreach events, clients educated, clients referred to prenatal program and healthcare utilization of prenatal care with VCC
Minimum Qualifications
- Bachelor's degree in related field
- Minimum two years outreach and community education experience or equivalent combination of education and experience
- Bilingual English/Spanish
- Valid California drivers license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record
- Availability and willingness to work evening and weekend hours, as required. Flexibility is a must.
- Comfortable with initiating conversation with various populations regrading prenatal program & services
Preferred Qualifications
- Minimum four years outreach and community education experience or equivalent combination of education and experience
Salary Range
- $23.00 - $25.00 per hour
