Job Description

SUMMARY

Supports the Referral and Care Coordinator Manager by ensuring the smooth and efficient day-to-day operation of the Referral and Care Coordinator department.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

  1. Interacts politely and as a team player with patients, staff, physicians and the public.
  2. Facilitate and participate in team huddles and trainings.
  3. Ensure workflow adherence by exhibiting a strong floor presence, monitoring departmental processes, monitoring jellybean completion within departmental standards, Referral and Care Coordinator processes, insurance incentive process and Joint Commission requirements.
  4. Provides and ensures that staff provide excellent customer service.
  5. Assists with the handling of patient escalations in the department.
  6. Regularly attend work, in-person, potentially traveling to multiple facilities within a single shift
  7. Creates ad-hoc reporting as needed for specialty or project items or quality committee follow-up items.
  8. Preps and gathers all documentation and materials for annual performance reviews of all staff reporting to the department management.
  9. Upkeep of Kronos records and requests (time off, timesheet corrections, timesheet approvals) for regional operations team.
  10. Assists in coordination of new hire training and transfers, orients new staff
  11. Provide refresher trainings and ensure all staff in the department are up to date.
  12. Determines the qualification and competence of staff.
  13. Conduct regular audits on referral and care coordinators completed tasks to ensure quality and continuity of care.
  14. Create work orders for IT tickets as necessary, reporting facilities to the Facilities team and schedule all equipment vendor maintenance.
  15. Complete Referral reconciliation and submissions.
  16. Provide support to the care coordinators when needed.
  17. Demonstrates the ability to function within Mountain Park Health Centers (MPHC) philosophy and its mission, fostering a relationship among and between MPHC, management and staff.
  18. Maintain staff call list and directories and provider productivity report.
  19. Review order request and partner with the Operations Team on supply ordering.
  20. Coach staff on appropriate Operations Management Team on corrective action for staff.
  21. Act on behalf of the Manager in his/her absence.
  22. Perform compliance completion audits.
  23. Assist in Coordination of participation in approved outreach events.
  24. Must be able to embrace differences among people and able to interact with internal staff as well as external contacts in a culturally competent and respectful manner.
  25. Other duties may be assigned as deemed necessary.

ADDITIONAL RESPONSIBILITIES
  • Promotes positive patient/guest relation in accordance with MPHC policies, providing a high level of quality in personal attention and service to patients and visitors.
  • Complies with MPHC Safety Policies and Procedures. Ensures compliance with governmental licensing and regulatory requirements where applicable. Conducts annual review of and complete/suggest on-going revisions of the Policies and Procedures specific to his/her department. Maintains familiarity with and abide by all established facility and departmental policies and procedures, noting changes when they are published/posted. Conducts himself/herself in a manner consistent with the Mission Statement and Values MPHC.
  • Observes and respects the confidentiality of information in regard to patients, visitors and co-workers employees including salary information, if exposed to any of the above referenced information in the course of his/her job function.
  • Demonstrates an understanding of facility organizational structure by utilization of appropriate channels of communication. Maintain consistent, timely communication regarding all facets of departmental activities with his/her co-workers, therefore, demonstrating good customer service skills with both external and internal customers.
  • After training and approved by Operations Director/Manager you will be responsible for drafting and issuing disciplinary actions.

EDUCATION and/or EXPERIENCE

Bachelors degree preferred; previous leadership experience required.

MATHEMATICAL SKILLS

Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

REASONING ABILITY

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

CERTIFICATES, LICENSES, REGISTRATIONS

CPR Certification.
