Job Description

ESSENTIAL FUNCTIONS FOR THIS POSITION

? Responsible for the tracking, coordination, and communication of outgoing member referrals

? Assembles information concerning members clinical background and referral needs

? Ensures that referrals are sent to the receiving provider in a timely manner

? Navigates multiple referral portals to ensure documentation is uploaded, accurate and is reflected in the medical record

NONESSENTIAL FUNCTIONS

? Ensures confidentiality of verbal and written information in accordance with HIPAA standards and Southwest Network policy, and adheres to the legal, ethical, and professional guidelines adopted by Southwest Network.

? Other duties as assigned.

EDUCATION and/or EXPERIENCE, LICENSURE and CERTIFICATION

? Associate degree or successful completion of a medical records certification program preferred.

? Must have a minimum of 1 year of experience in healthcare, preferably working directly with the collection and disbursement of medical records.

? Must maintain a valid Arizona state fingerprint clearance card.

? Must be trained in CPR, First Aid and Therapeutic Options ? Must have and maintain a valid Arizona drivers license.

? Must be at least 21 years of age.

KNOWLEDGE, ABILITY and SKILLS

? Strong understanding of medical terms

? In-depth understanding of HIPAA guidelines ? Excellent written and verbal communication skills.

? Ability to maintain a high level of integrity and confidentiality of medical information.

? Must be willing to use interactions with members to help enhance their recovery process by making them feel welcome, reducing feelings of stigma and fear, and by demonstrating sincere respect, compassion, and friendliness.

? Must be competent in the use of electronic medical records software, as well as in the use of a computer, printer, copier, and telephone. Preferred typing skills of 25 to 40 WPM. Occasional use of fax machines, security systems, and other office equipment.

? Must be highly proficient in the use of Microsoft Office software, including Outlook, Word, and Excel.

? Must be able to effectively use the internet and various web browsers.

WORKING CONDITIONS and DRIVING

? Travel between Southwest Network sites as well as in the community is required. Must have a registered and reliable vehicle that meets Arizona state law standards and meets the insurance standards of Southwest Network.

? Work is performed in the community as well as an office setting; must be able to move about; occasionally exposed to outside weather conditions.

? Must be able to remain in a stationary position for long periods of time and position self to reach items under or above desks, in storage, or in areas not easily accessible; some lifting also required.

? Evening or weekend hours required on an as-needed basis.
