JOB SUMMARY
This position focuses on health care for individuals, families, groups, communities and populations which occurs in primary and specialty care outpatient clinics. Addresses in partnership and collaboration with other health care professionals the patient's wellness, acute illness, chronic disease, disability, and end -of-life needs. Responsible for patient advocacy, coordination of nursing and other health services, implementation of nursing services and continuity of care. Provides patient education when needed.

ESSENTIAL DUTIES/EXPECTATIONS
  • Systematically collects focused data relating to the health needs and concerns of a patient, group, or population.
    • Consistently demonstrates the ability to assess the patient's physical condition including pain level R/T the chief complaint for the encounter. Collects subjective and objective data relevant to the patient's condition
    • Documents the information and data collected in a retrievable, understandable, and readable format. Accurately completes the flow sheet for each encounter. Reviews and clarifies medications and documents on medication profile.
    • Completes an appropriate assessment when triaging a patient and documents appropriately in the patient's EHR.
    • Synthesizes available data, information, and nursing knowledge relevant to the presenting health situation, and/or anticipated needs.
  • Analyzes the assessment data to determine the diagnostic statements for health promotion, health maintenance, or health-related problems or issues.
    • Consistently monitors the return of lab, diagnostic, etc. results on a daily basis.
    • Informs ordering provider of abnormal labs/orders taken and patient if notified. If ordering physician not available results are actively directed to an available provider.
    • Ensures all necessary lab results, etc. and referral forms are available at time of appointment.
  • Coordinates the delivery of care within the practice setting and across health care settings. Employs strategies that promote individual and community wellness.
    • Ensures safe patient care in accordance with the Wisconsin Nurse Practice Act.
    • Aware of and utilized JACHO and HIPAA standards in the delivery of safe patient care.
    • Responds and participates in emergency situations quickly, calmly, and safely, and participates in mock drills.
    • Provides reliant information across levels of care within one or more health care systems and when the patient's care is transferred between and among different specialties and/or within one or more organizations. Serves as the point of contact for coordination of care across multiple service agencies and / or health care organizations.
    • Uses teaching methods and approaches appropriate to the situation that recognize the age, gender, race, ethnicity, diversity, and cultural norms of the individual patient.
    • Supports patients and caregivers in developing skills to promote, maintain, or restore health, such as healthy lifestyle tips, risk-reducing behaviors, age and developmental needs, daily living activities, and preventative care.
  • Evaluates progress toward the attainment of identified goals.
    • Evaluates the effectiveness of care given and appropriateness of care.
    • Communicates urgent needs or undesired outcomes of treatment to the provider.
  • Actively engage in organizational initiatives that create and maintain an internal environment that is safe, hazard-free, ergonomically correct, confidential, and comfortable for patients, visitors, and staff.
    • Actively participates in clinic committees
    • Belongs to / involved in professional organizations.
    • Participates in continuing education beyond what is mandatory.
    • Actively functions as preceptor / charge nurse when appropriate
    • Participates in network / community programs
    • Attends 75% of all unit meetings
    • Actively participates in clinic PI projects
  • Evaluates own nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules, regulations, and needs of specific patient population being served.
    • Maintains clinical competencies and demonstrates proficiency in specialty area of practice
    • Demonstrates competency through review of work performance, verification of credentials, participation in ongoing education and updates of procedures, equipment, regulatory guidelines and statutes and the reassessment of practice-specific competency.
EDUCATION/EXPERIENCE REQUIREMENTS
  • Graduate of an accredited school of nursing required, and current unrestricted Wisconsin RN license required.
  • None required; at least one year medical/surgical nursing preferred.
  • CPR certification required. Excellent communication and organizational skills required
HOURS
  • 24 hours per week 1st Shift (8:00am - 4:30pm)
  • Floats between Manitowoc and Sheboygan Clinics
