Job Description

Ideal candidates will be responsible for providing skilled nursing care to patients while adhering to HIPAA compliance standards, and all state and federal regulatory requirements for home health care. The candidate should be able to provide patient-centered care in a home health care setting. The successful candidate will recognize elderly patients' needs, develop interventions, and collaborate with a dynamic care team, case managers, and staff, to promote the health and well-being of patients who desire to maintain their independence and age safely in the comfort of home.

Successful candidates will receive a hiring bonus up to $5k upon meeting eligibility and qualifying criteria. Dominican Home Health Agency offers a generous, competitive total compensation package including health and dental insurance, STD & LTD, Life, and a 403(b) plan, paid training & continuing education, and licensure renewal fees. Mileage reimbursement biweekly, and expenses, and other allowances, as permitted.

Education requirements

RN with active licensure, BSN preferred

Experience requirements

One year of home health or nursing in skilled facility

Qualifications

Must be a Registered Nurse, preferred BSN with current licensure
CPR, ACLS, and/or BLS Certification
Knowledge of HIPAA compliance and EHR maintenance
Candidate should demonstrate proficiency with EHR management and timely charting
Ability to deliver coordinated care as a health care team member and available to work Monday-Friday, 8-5PM - *Flexible schedules available*

Responsibilities

Assure quality care by adhering to professional standards of nursing practice
Supervision of CNAs and report to Director of Clinical Services
Adhere to HIPAA, state, and federal regulatory requirements
Must be able to travel daily to patients' homes as assigned in geographic area
Work closely with Director of Clinical Services to provide quality nursing care

Skills

Experience with EHR management
Knowledge of home health care
Proficiency with scope of practice and provision of skilled care for geriatric health populations
Ability to work independently and exercise sound judgment
Strong communication skills

Work hours

Flexible schedules available M-F 8:00-5:00 or hours can be adapted to availability and patient needs
