Summary: Administers exceptional skilled nursing care to patients on an intermittent basis or through private duty services in their place of residence. This is performed in accordance with physician orders and the patients plan of care under the direction and supervision of the RN Case Manager and the Director of Clinical Services. Services are furnished in accordance with Agency policies and procedures. This is a safety sensitive position.

Primary Responsibilities:

  • Provides services in accordance with the plan of care and provides total patient care as needed.
  • Responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.
  • Completes the initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es) and admits to appropriate level of service/care.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Completes and submits OASIS assessments, reassessments, and transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Uses health assessment data to determine nursing diagnosis.
  • Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
  • Coordinates and collaborates with all services associate with patient care.
  • Informs appropriate personnel of changes in the condition and needs of the patient.
  • Counsels and educates the patient and family/significant others in meeting nursing and related needs.
  • Attends all in-service programs as required.
  • Understands and adheres to established Agency policies and procedures.
  • Processes orders and notifies physician of patient needs and changes in condition.
  • Completes certification/recertification orders and discharge summaries.
  • Refers to Physical Therapist, Speech language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
  • Supervises and teaches other nursing personnel as assigned.
  • Conducts patient care conferences on patients assigned to his/her care.
  • Participates in peer review and quality assessment and performance improvement projects as assigned.
  • Participates in utilization review of medical records as assigned.
  • Must perform duties following safety standards required for this position including but not limited to use of PPEs.
  • Assumes responsibility for personal growth and development. Maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.

Qualifications

Minimum Qualifications:

  • Must be at least 18 years of age.
  • Must have a high school diploma or the equivalent.
  • Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
  • Two years nursing experience.
