The purpose of the Residential Case Manager position is to provide case management and coordination services to clients in residential treatment and their families to include direct support, crisis intervention, transportation, screening, assessment, and treatment planning.

Essential Responsibilities:

  • Serve as a point of contact and ensure ongoing collaboration with the clinical team and other involved parties as appropriate. Collaboration may include a persons health plan, referral source, family, behavioral and general medical and dental health care providers, community resources, and other involved supports including educational, social, judicial, community and other State agencies. Involve natural supports as preferred by the client.
  • Facilitate, schedule, and provide follow up on Child and Family Team Meetings/Adult Recovery Team Meetings. a. Work collaboratively with the clinical team to ensure that all services prescribed in the individualized service plan are implemented. b. Make referrals to collaborating partners and other community services, as client needs and service plan indicate. c. Conduct coordination of care activities related to continuity of care between levels of care and across multiple providers, services and supports.
  • Facilitate the residential admission and discharge processes including: processing incoming referrals and facilitation of screenings, admission, clinical assessment, and discharge.
  • Provide direct support, as needed, in order to support the client in maintaining stability in their placement and in developing skills as outlined in their individualized service plan.
  • Deliver at least 40 hours of work per week, with a goal of 30 hours of direct client contact. Productivity standards established at a minimum of once a year, and as needed, due to contract changes.
  • Maintain all client records in current status and in accordance with agency policies and procedures, as well as health plan, state, and federal requirements. Produce complete, timely, thorough, quality clinical documentation of all services rendered, both billable and non-billable, as required. The documentation must include sufficient information about the activity being documented and the clients participation in, and response to, the activity.
  • Maintain the persons comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning).
  • Provide transportation to consumer as appropriate and determined by the residential team.
  • Attend regular scheduled weekly supervision meetings. Seek out additional supervision as needed.
  • Participate in all regularly scheduled administrative, in-service, and department staff meetings.
  • Recommend to the Clinical Coordinator or Department Manager changes that would improve service delivery. Implement changes and new programming as directed.
  • Evening and weekend hours may be required to accommodate the schedules of children, families, and adults. Attend meetings and trainings as requested.
  • Provide continuous evaluation of the effectiveness of treatment through the ongoing assessment of the person and input from the person and relevant others resulting in modification to the service plan as necessary.
  • Adhere to the agency's policies and procedures. Comply with the agency's standard of care for service delivery. Assist with Quality Management, Quality Assurance, and Utilization Review.
  • Perform additional related duties and participate in agency fund-raising activities, as requested

  • Strong computer skills including experience with Microsoft Office.

Education and Experience:

  • Bachelors degree in Psychology, Social Work, Family Studies, or other behavioral health field. OR 4 years of experience of direct behavioral health services to meet BHT requirements.
  • Experience working with substance abuse and co-occurring disorders preferred.

Physical Demands and Work Environment:

  • The typical work environment will be working indoors or community-based work, working out in the community.
  • Employees must be able to lift up to 15 pounds at times.
  • Occasional travel to training and out-of-state locations may occur as-needed.
  • Possibility to work from home may be necessary.
