We have a new and exciting WORK FROM HOME opportunity on our CVS Health Accordant Team! Everyone values good health. Accordant's care management programs bring members and their health care providers together to bridge the health care experience.
Resource specialists are members of the Health Management team. The Resource Specialist participates in resource identification to promote the well-being of program participants. They function in collaboration with all members of the healthcare team to identify community and government resources to meet a member's need.
As a Resource Specialist you will collaborate with other team members to obtain the best understanding of member needs/situation. You will function as a resource to other team members to provide the best understanding of potential assistance for members and to members in obtaining information for the best available assistance with resources. In this role you will also identify funding sources, utilizing national foundations as a member resource where available.
The preferred candidate will have gathering skills to assist with identifying available resources the member is eligible to receive. The candidate should possess a current working knowledge and understanding of community and national resources centers. Effective communication with multi-setting resource agencies to investigate the availability of resources and member qualifications is critical and the preferred candidate will work independently and multitask effectively to meet member and client needs.
To be successful in this role you must have demonstrated ability to build positive relationships and the ability to adapt to change, plan, organize and evaluate effectively. You must also have effective written and verbal communication skills to support multi-modal communication strategy and aptitude for PC skills, proficiency with Microsoft and web based applications.

Required Qualifications
-2+ years of prior related work experience in Social
Work or a community related discipline demonstrating the ability to plan, organize and evaluate patient well-being effectively.
Schedule: Monday thru Friday 8am - 5pm, with 1 flex day out of the week. The hours for the flex day would be a choice of 1) split shift 8am - noon and return for 5pm - 9pm or 2) 12pm - 9pm shift.
Preferred Qualifications
-Healthcare certificates or licensure
-Advanced experience in a health care community agency
-Intermediate Microsoft Office Skills
-Advanced Degree/Masters in related field
-Other Certificates or Licenses - LCSW and 2 years experience in a community agency.
Education
-Bachelor Degree in Social Work or related field
