Pay Range

$28. 85 - $40. 62/hr

Responsible for providing a full range of personalized care in the home hospice setting, which may include observation and assessment, direct care and management and evaluation of the care plan to terminally ill patients. Responsible for disease, pain and medication management, as well as, wound care, infusion and various other nursing tasks. Assumes responsibility and accountability for facilitating, communicating and collaborating with both the healthcare team and the patient/family to identify and meet the physical, emotional and spiritual needs of the patient. Facilitates patient / family identified goals for care. Promotes quality end of life care through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrates personal accountability for relationship-based care, organizational mission and core values.

What You Bring/Job Requirements

Qualified external applicants eligible for a $15,000 sign on bonus! Cannot be a current or recent Centura employee to qualify. Minimum of 1 year experience required. Must be hired into a Full-Time or Part-time position; PRN positions are not eligible.

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:

Graduate of an accredited School of Nursing BSN Preferred

Six months of recent nursing experiencePrevious hospice, home care or oncology experience, preferred

Requires knowledge in nursing, evidence-based practice methods and person-centered care concepts

Current state RN license for applicable state of employment or current RN license from a participating state in National Compact Licensure

Current Basic Life Support (BLS) certification from American Heart Association Valid driver's license

Schedule: Hours will be 4 - 10 hour days. Some weekends required.

This is a hospice admissions position and the associate will be out in the field at hospitals, homes, etc consenting for hospice and admitting patients in the home.

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
