SUMMARY:

The Clinical Sepsis/Pneumonia Coordinator, provides Knowledge Management leadership and oversite of the Sepsis and Pneumonia Improvement Programs and assumes responsibility for coordination of care for pneumonia and sepsis patients. Utilize continuous quality improvement to assure quality of care, facilitate protocol development and revision, and provide ongoing education in both the clinical and community settings. Responsible for facilitating performance improvement efforts, including education, collection and use of data to report on results, as it pertains to the screening, assessment, and management of patients with Sepsis and Pneumonia. Provides comprehensive clinical coordination of data collection and process improvement for patients throughout Medical Center Health System. Utilizes continuous quality improvement to assure quality of care, facilitate protocol development and revision, and facilitates evidence-based practice. Provides ongoing education in both the clinical and community settings. Provides comprehensive clinical coordination of data collection and process improvement for patients throughout Medical Center Health System.Responsible for the planning, development and quality improvement plans for the sepsis and pneumonia programs. Collaborates regularly with other members of the Team and/or Emergency Physicians/Critical Care Physicians to ensure continued system focus and improvement of care.

ESSENTIAL FUNCTIONS:

Coordinates the Sepsis and Pneumonia Committee and is responsible for planning and implementation of the sepsis and pneumonia programs, achieving improved outcomes from the work and support of this role.

Responsible for developing this role as an expert in Sepsis and Pneumonia protocol implementation within Patient Care Services. Serves as the point of contact for Primary Coordination status for TMCH employees within the health system.

Provides coordination of CNS staff to lead EBP work.

Collaboratively develops and oversees the implementation of strategy for evidence-based practice in alignment with TMCH Mission, Vision and Values.

Creates a culture of inquiry, encouraging nurse engagement in EBP.

Promotes nurse involvement in relevant organizational research and EBP activities.

Identifies opportunities for internal and external funding for planned nursing EBP initiatives. Coordinates and/or participates in the writing of grant applications and management of the implementation of awarded grants, as appropriate.

Develops and oversees the maintenance of policies and procedures related to EBP.

Provides consultation to internal staff and external agencies seeking advice and approval for EBP projects at TMCH.

Provides consultation and mentorship to internal staff regarding the conduct of evidence-based practice, and dissemination of findings.

Builds infrastructure and oversight for the dissemination of EBP findings.

Provides oversight for the activities and responsibilities for EBP, including review of proposals for EBP.

Collaborates with nurses, advanced-practice nurses, clinical nurse educators, and other organizational leaders, to provide training, consultation, and mentorship for evidence-based practice projects.

Provides coordination for EBP protocols for staff.

Acts as Principal Investigator for nursing EBP studies as applicable.

Builds relationships (partnerships, support) internally and externally for EBP opportunities for nurses.

Contributes to the activities of organizational initiatives, committees, and councils related to EBP.

Promotes collaborative EBP efforts across disciplines and settings.

Performs related duties as assigned.

MINIMUM QUALIFICATIONS

EDUCATION: Bachelors Degree in Nursing is required, Masters is preferred.

EXPERIENCE: Five (5) years of clinical nursing experience with experience conducting various EBP initiatives.

LICENSURE OR CERTIFICATION: Current RN licensure permitting work in the state of Arizona, Basic Life Support (BLS), and Advance Cardiac Life Support (ACLS) required. Additional certification may be required for some units.
