Position Summary : Under general supervision of the Senior Nurse Manager Case Management (SNMCM) the Complex Care Case Management RN, Supervisor (CCCMRNS) will oversee the Complex Care Case Management RN team. This position is treated as FLSA
Distinguishing Features: River People Health Center is seeking a dedicated Registered Nurse (RN) to join our team as a Complex Care Case Management RN, Supervisor (CCCMRNS) The CCCMRNS will play a crucial role in coordinating and managing complex patient care, ensuring seamless transitions between healthcare settings, and advocating for the needs of our patients and providing patient education and disease management. This position requires strong clinical skills, effective communication, and a passion for improving patient outcomes.

Examples of Tasks
The Complex Care Case Management RN, Supervisor oversees the care of the patient with complex healthcare needs as they move thought the health care continuum with multi-morbid conditions. Supervises staff in day-to-day functions, including training and developing the case management and care coordination staff to accomplish program goals and objectives. Assists in the development, maintenance, and implementation of metrics to appraise the productivity and quality of staff performance. Supervises the management and care coordination of identified high-risk patients to reduce recurrent and unnecessary hospital admissions, readmissions, and emergency room visits. Supervises the management and care coordination, patient navigation with the clinic and outside providers. Works in close collaboration with Multi Care Team and contracted physician groups who conduct assessments to establish members' care needs. Supervises the process to promote disease management, education, training, and other clinically based activities to coordinate care among providers, patients and the community Patient education and health literacy Monitor staff performance, including performing case reviews and structured observations. Hold weekly supervision meetings with case management teams. Multidisciplinary Care Coordination: Follows established policies, procedures and standing orders to coordinate care for high-risk patients that are at risk for poor health outcomes, frequent emergency room visits and hospital readmissions. Oversees communication and collaboration with the Case Manager RN, physicians, social workers, care team members and other care giving disciplines with the patient/family to develop and implement care plans for the patient. Monitors the clinical outcomes to avoid complications and unanticipated variances. Respect confidentiality of all patients and follow organizational, state and federal policies in accordance with HIPAA. Acts as a resource to Chronic Care Case Manger RN to contact with the patients with a focus on behavioral goal setting and patient-centered support planning. Identifies patient resource needs, provides information on possible resources and referrals to patients. Supports the Chronic Case Manager RN with network participation, care with specialty networks, care with medical providers, and transfers to alternate levels of care using knowledge of benefit plan design Assists the Chronic Care Case Manager RN to recommend services for patients using care alternatives available in the community. Identifies potentially unnecessary services and care-delivery settings and recommends alternatives by analyzing clinical protocols Maintains professional knowledge and proficiency in nursing through continuing education, staff meetings, training and conferences, etc. Miscellaneous: Performs other job related duties as assigned to maintain and enhance departmental operations.

Minimum Qualifications
- Education and Experience: Active Arizona Registered Nurse license from an accredited college or university and certification as a Registered Nurse (RN) required.
- Arizona Registered Nurse license required.
- Minimum 3 years' work experience as a Registered Nurse
- One year of clinical nursing experience in an outpatient primary care setting and/or as an inpatient case manager RN.
- Minimum one year of RN case management experience in a primary care setting.
- Continuing education and professional development in advancing chronic complex care.
- Experience working with Native American Communities preferred.
- CCM Certification (Certified Case Manager) preferred, or the ability to obtain certification within six to nine months of hire.
- 1 year of leadership experience preferred.
- Equivalency: Any equivalent combination of education and/or experience that would allow the candidate to satisfactorily perform the duties of this position, will be considered.
- Underfill Eligibility: An enrolled Community Member whom closely qualifies for the minimum qualifications for a position may be considered for employment under SRPMIC Policy 2-19, Underfill.
