What Success Looks Like In This Job

Youth in Transition Social Caseworker III position specializes in the adolescent population, ages twelve to twenty-one years old, who have the Permanency Goals of Other Planned Permanent Living Arrangement (OPPLA) that include long-term foster care and emancipation. YIT workers will also work with the teen parent population and older adolescent youth with goals of adoption. YIT has a goal to have youth in the most family-like/ least restrictive setting that meets their needs, to empower the youth to succeed in their personal goals, build permanent connections, and achieve legal permanence for the youth.

Examples of Duties for Success

  • Completes thorough assessments of family and youth for individual/community risk, services, and resources.
  • Locates appropriate placement and monitors the safety, permanency, and well-being of the youth.
  • Guides/develops treatment plans and ensures appropriate treatment needs are being met on a continual basis by various service providers.
  • Documents case activities while complying with all federal and state mandates and guidelines.

  • Provides face-to-face contact with clients, as required by regulatory standards and agency policy (may sometimes include short/long distance travel in-state and/or out-of-state travel).
  • Participates with interdisciplinary consultation and collaboration on a regular basis.
  • Prepares court reports, attends court hearings, testifies in court hearings and proceedings, consults with attorneys regarding legal aspects of cases.
  • Coordinates and maintains professional communication with collateral agencies.
  • Engages the youth in finding permanent connections, re-engages families when applicable, participates in Family Engagement Meetings and Permanency Roundtables.
  • Performs related duties and responsibilities as required.
Qualifications for Success

  • Knowledge of adolescent development, family dynamics, child abuse and neglect dynamics, youth cultures, crisis intervention, mental health, and substance abuse issues.
  • Skills and sensitivity in working with issues of clients from a variety of cultural, ethnic, and socio-economic backgrounds.
  • Knowledge of various computer systems to complete mandated case management reports and documentation.
  • Ability to build professional relationships with various community resources.
  • Ability to prioritize tasks and make informed decisions.
  • Ability to utilize critical thinking skills and the ability to work difficult or complicated cases.
  • Ability to work cohesively with other members of a team.
  • Ability to communicate clearly and concisely, both orally and in writing.
  • Establish, maintain, and foster positive and harmonious working relationships with those contacted in the course of work.

Experience:

  • One (1) year of full time professional social casework experience, acquired after the degree in a public or private social services agency, in an area appropriate to the position.

Education and Training:

  • Bachelor's Degree in one of the human behavioral science fields.
  • An MSW may substitute for all of the required experience.
  • An educational waiver is available for those individuals who do not meet the education requirements to be a social caseworker and are currently enrolled in a bachelors program from a higher education institute. A bachelors degree must be obtained within one (1) year from hire date with related coursework to job duties as outlined in 7.603.1 (click here for more info). If a degree is not obtained within one (1) year from hire date, employment with the agency may be impacted including, and up to termination. Employee will be responsible for updating the hiring manager every six (6) months of their education progress.
  • Proof of education, (degree or transcripts), and license (if candidate has received one) are required.

License or Certificate:

  • Colorado Child Welfare Caseworker certification required and must be completed within three (3) months of employment, through the Colorado Training Academy (State Child Welfare). Certification through another State will require proof of completion and may substitute for all or part of the training required by Colorado.

  • Possession of a valid driver's License and proof of automotive insurance.

Background Check:

  • Must pass a criminal background check.

Other:

  • Must utilize a personal vehicle to get to many appointments in the local community and surrounding areas within Colorado.
