Social Worker- Bilingual

Boro Park Center is seeking a full time Social Worker for our Skilled Nursing Facility in Brooklyn, NY. The ideal candidate will have experience in a long term care / sub-acute setting, speak Mandarin or Cantonese, and have long term care experience.

DUTIES:

• Maintaining and/or improving residents ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and Maintaining individualized care plans
• Participating in the planning of the residents admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

REQUIREMENTS:

• Master's Degree in Social Work, Mental Health, or Related Field.
• Bilingual- Mandarin or Cantonese Preferred
• Long Term Care / Subacute Experience Required
