Position Description
This position is responsible for assisting patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care; or may also be responsible for working with patients who are emotionally or mentally disturbed, collaborating with psychiatric and allied providers teams in diagnosis and treatment plan.
They work closely with physicians and nursing staff to further their understanding of significant social and emotional factors underlying patient's health problem. Work is performed under the general direction of the Director of Social Services, and requires frequent contact with representatives of other social service related agencies.

Major/Essential Functions
  • Provide case management and support services to pediatric patients and their families within the Center for Superheroes, Foster Care clinic and Medically Complex Care clinic.
  • Assess the needs of each individual for social services.
  • Screen and assist for financial resources to assist with cost of medical care.
  • Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem.
  • Be liaison between patient, family, medical staff and outside resources to assist with continuation of medical care.
  • Serve as information and referral source if needed.
  • Receive and review forms from all providers that may require a physician's signature and prepares patient histories, service plans, and reports.
  • Participates in planning for improving health services by interpreting social factors pertinent to development of program.
  • Make home visits on an as needed basis, and provides general direction and supervision to workers engaged in clinic home service program activities.
  • Helps patient and family through individual or group conferences to understand, accept, and follow medical recommendations.
  • Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability.
  • Provides service plans to restore patient to optimum social and health adjustment within patient's capacity.
  • Link to needed community resources.

Required Qualifications
Education:
A minimum of a Master's degree in Social Work from an accredited Graduate School of Social Work.
License/Certification:
Licensed as a Social Worker by the Council on Social Work Education.
