Primary Purpose:
Coordinate activities for children with disabilities focused on improving the academic and functional
achievement of the child with a disability, to facilitate the childs movement from school to post-school
activities, based on the childs needs considering the childs strengths, preferences, and interests.

Qualifications:
* Texas Teacher Certification with endorsement or specialization in Special Education
* Masters degree in the field of education or related field preferred
* 3 years successful teaching experience
* Knowledge of special education policies and procedures including state and federal
compliance guidelines

Duties and Responsibilities:
1. Competently administer vocational assessments as needed/required
2. Collect and interpret assessment data for use in student placement and instructional programs
3. Design a coordinated set of activities designed to facilitate the childs movement from school to
post-school activities as required by state and federal law, and local policy and guidelines
4. Participate in Admission, Review, Dismissal (ARD) meetings as requested
5. Facilitate student involvement in the transition to life outside the public school system
6. Facilitate appropriate involvement in the childs transition by the childs parent and other
persons invited to participate in ARD meetings
7. Facilitate the implementation of appropriate transition goals and services as required
8. Maintain a database of student records as needed for positive student outcomes
9. Consult with school administrators, faculty, and other personnel relative to the curricular
offering which meets the abilities, interests, and needs of students
10. Train and support Co-op and district/campus personnel in the Transition process
11. Serve as a resource for vocational programs and activities, including 18 services
12. Maintain accurate student records in compliance with federal, state and local guidelines
13. Conduct audits of compliance folders with a focus on guidelines for SPP Indicator 13 and 14
14. Organize and disseminate information relating to Indicators 13 and 14
15. Complete TEA data entry for SPP Indicators 13 and 14
16. Attend training related to Transition and ensure special education staff and district/campus
personnel are informed
17. Develop and provide staff development in the area of Transition
18. Ensure district compliance with state, federal, and local laws and policies regarding Transition 19. Establish and maintain relationships with community agencies and other resources
20. Facilitate student referrals to agencies and organizations as required
21. Organize an Annual Transition Fair with the purpose of assisting students, parents, and school
personnel to facilitate coordination of student services with agencies and other organizations

Mental Demands:
Ability to communicate effectively (verbal and written), interpret policies and procedures, reason, read,
understand verbal instructions, analyze, differentiate, coordinate, compile, instruct, and maintain
emotional control.

Physical Demands:
Ability to lift and/or carry moderate weight, sit, stand, walk, reach, repetitive hand motions, hear, speak,
and frequent travel within the cooperative/districts and occasionally outside the cooperative boundaries
and/or county. May experience prolonged and irregular works hours and work with frequent
interruptions.
