    Summary:
    Assist young adults with disabilities as they transition from high school to the community. Provide support or direct instruction in the community that address skill development in young adults in the area of employment and pre-employment.

    Responsibilities:
    With guidance from the transition program coordinator and support from the transition team:
    -Develop and implement individualized student training in areas of employment, community connections, daily living skills, interpersonal skills and recreation skills
    -Collaborate with adult service agencies to provide a smooth transition of services for students
    -Team with parents, support their needs and integrate their goals for their children into students' programming
    -Mentor, support and teach young adults with a variety of abilities and disabilities to become more competent, independent adults
    -Work with employers to develop job experiences, on-the-job training and paid employment; provide job coaching and job follow-along
    -Train and supervise paraeducators as job coaches and community-based trainers
    -With assistance, develop individualized student goals, attend staffings, write progress reports, maintain written documentation, network with BVSD staff and contribute to program projects
    -Follow safety procedures, maintain confidentiality, and follow federal, state and district laws and procedures concerning special education and transition services

    Qualifications:
    Required:
    -High school diploma or equivalent
    -Completed and submitted BVSD online application
    -Communicate (read, write and speak) in English, must pass English component of interview process
    -Must have insured automobile and be able to drive from one site to another during the day

    Preferred:
    -Bilingual ability (Spanish/English)
    -Experience working with youth with a variety of disabilities, adult service agencies and community resources, or similar experience
    -Computer skills, training or teaching experience and flexibility in scheduling work hours
    -Good public relations skills
    -Variety of life experiences which contribute to understanding youth, youth with disabilities, individualized instruction and how to support youth in gaining independence
    -Should be a self-starter, as well as a team-player
    -Ability to work well with students, parents, co-workers, adult agencies and employers
    -Experience or willingness to learn how to develop jobs and to teach on-the-job skills

    Salary Information:
    -Salary Placement varies according to experience and education.
