Eastern Plumas Health Care is a small, non-profit, critical access hospital district, providing comprehensive medical services to Plumas County since 1971. We operate a 9 bed acute care hospital at our main Portola campus, which includes a 24 hour, physician staffed, emergency room and ambulance service. Our Skilled Nursing Facilities, located at the Portola and Loyalton campuses, provide a home to our senior community with long term medical needs and a place for rehabilitation for those who are recovering from physical injury or illness. Our Therapy and Wellness Center offers Outpatient Physical, Occupational, Hand and Speech Therapy services with state of the art equipment and an experienced team.

POSITION SUMMARY:

The Physical Therapist performs a wide variety of patient care activities under the supervision of Lead Physical Therapist.

PRINCIPAL ACCOUNTABILITIES:

  • Evaluates patients and creates effective plans of care for adults and adolescents with orthopedic, neurological, cardiovascular and other diagnoses.
  • Restores patients function, alleviates pain, and prevents disabilities by planning and administering medically prescribed physical therapy.
  • Applies best practices to treat and progress each patient to the most favorable outcome.
  • Documents patient care services by charting in-patient and department records.
  • Evaluates and records patients progress, modifying treatment plans, and trying new treatments if required.
  • Instructs patients in proper in-home exercises and therapies.
  • Educates patients and their families regarding their diagnosis and the treatment plan prescribed to achieve the best outcome."
  • Maintains accurate patient treatment records and current project notes.
  • Submits billing via EMR for each patient treated in accordance with established policy.
  • Participates in staff meetings, in services and other educational opportunities.

EDUCATION/EXPERIENCE:

Must possess or be eligible for a license from the CA Board of Physical Therapy. 1 year of previous experience in an outpatient physical therapy setting is required.
