SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Participants will be for the most part senior citizens and United States Armed Forces Veterans.
* Experience and a clear understanding of telehealth services, electronic medical record and remote patient monitoring knowledge is important.
* The selected candidate is required to be able to identify and perform patient health assessments, creating strategies to improve participants health and well-being, and where necessary, introduce techniques for promoting improved health and disease prevention.
REQUIRED EXPERIENCE & EDUCATION FOR CASE MANAGER
* Must be a licensed Nurse Practitioner in the State of Michigan.
