Our Client is seeking Texas licensed Geriatric Case Managers that will provide services in the Houston area.
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Licensed Clinical Social Workers are needed for part-time or full-time tele-health opportunities in Texas to provide individual therapy and behavior management support to older adults as part of a multi-disciplinary team comprised of a MD, in family medicine, Nurse Practitioners, Psychologist, LPCs, Medical Assistants, and Companion sitters.
* Through the IOP model, the Case Manager will provide remote services to the geriatric population that is experiencing mild cognitive decline and early onset demetia.
* Therapies will include art, music, exercise, nutritional, and reminiscence.
REQUIRED EXPERIENCE & EDUCATION
* Master's Degree in Social Work
* LSW or LPC licensed in the state of Texas.
