OBJECTIVE AND POSITION SUMMARY
Assess the needs of identified clients to create treatment plan goals focused on helping clients be successful living in home/community setting. Provide brief, solution-focused therapy. Utilize strength-based, and empirically sound interventions to help clients reach treatment goals. Collaborates as needed with appropriate agency staff and community partners regarding client care issues. Completes clinical documentation, including assessments, treatment plans, progress notes, monthly reports, and discharge summaries (as required).
MINIMUM QUALIFICATIONS AND COMPETENCIES
A Master's Degree in counseling, social work, or related field plus 2 to 5 years of clinical experience providing counseling. Minimum of one year working with adolescents. Must be licensed (LAC, LASAC, LMSW, LPC, LISAC, LCSW, LMFT).

PRIMARY DUTIES AND ESSENTIAL FUNCTIONS

Bringing compliance issues to the attention of management
Successfully completing regulatory training requirements periodically.
Working collaboratively in all facets of position requirements and support agency goals
Provides weekly individual, group and family therapy to adolescents. Sets up a regular schedule for individual and family sessions, to be held as needed and agreed upon. Provides empirically sound intervention techniques.
Completes a brief strengths-based psycho-social assessment of all new clients to determine focus of therapy.
Creates treatment plan goals based on the assessment, with input from the client, client's family, if appropriate, and external Team Members.
Develops active discharge plan for each client and discusses progress towards goals with client, family members, if appropriate, and external Team Members
Provides resources for clients to community programs or services that would supplement treatment and counseling, both during placement and when transitioning out of placement.
Provides excellent customer service and case management, including timely updates and responses to all internal and external requests for information, including agency staff and outside team members. Maintains regular contact with appropriate staff members to give / receive feedback on client's progress and new or emerging needs.
Responsible to maintain all client records and documentation according to licensing requirements, including completing assessments, treatment plans, progress notes and discharge summaries in a timely basis. Submit completed paperwork to supervisor.
Conform with all safety rules and use all appropriate safety equipment.
Performs all other related duties, as assigned. Participates on committees and special projects and seeks additional responsibilities.
Effective January 3, 2022, all CPLC Employees and all Service Providers, are required to be Fully Vaccinated against COVID-19. Newly hired employees, who are not Fully vaccinated at the time of hire, are expect to begin a vaccination series immediately upon hire. CPLC recognizes medical and religious exemptions to the COVID-19 vaccination requirement, in accordance with applicable law and upon approval by CPLC

- Must have and maintain a valid Arizona Driver's License with an acceptable driving record, automobile insurance coverage and have access to an automobile.
