Overview

Vista Community Clinic is a private, non-profit, multi-specialty outpatient clinic providing care in a comprehensive, high quality setting. Responsibilities
  • Establish, maintain and develop relationships with patients
  • Ensure appropriate intake steps are followed, including eligibility, assessment of needs, collecting patient data, enrolling in program, developing care plan, and other steps as required by department guidelines
  • Complete all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards
  • Efficiently track, monitor and actively manages assigned patient cases to ensure coordination of care
  • Operate a computer using word processing, spreadsheet and database software applications, and operate other standard office equipment
  • Assist in preparing reports
Qualifications

Minimum

  • High school graduate or equivalent
  • Bilingual English/Spanish

Preferred

  • Medical Assistant experience preferred working with the medical home concept or for a non-profit community health clinic
  • Certificaton/minimum two years' education in chronic disease/health

Required Skills/Knowledge/Abilities

  • Excellen verbal and written skills necessary for communication with patients/clients, clinicians and other staff
  • Ability to interface with all levels of personnel in a professional manner
  • Experience/familiarity with computers and proficient in Microsoft Office products, specifically Word and Excel
