Job Description

New Alternatives Transitional Housing Program-Plus is a participant-driven community housing program geared at combating homelessness through supporting youth in their goals to attain skills, employment experience and education as they journey toward independent living. The primary role of the case manager is to provide consistent oversight and guidance to support youth in program compliance and progress towards stated personal goals as they transition into adulthood.

The case manager will meet with a caseload of 10-12 youth on a weekly basis and be consistently available to support residents with challenges of navigating adult life.

Utilize experience implementing concrete behavioral interventions and understanding of the emotional and developmental needs of this population to build rapport, foster long term relationships with residents and their support groups, and address 6 life domains and outcome measures of: Housing stability, education, employment, permanency, health/mental health, and living wage.


Job Duties Include:

  • Monitor weekly tasks/ensure follow through with weekly schedule and program expectations. 
  • Model professional boundaries and professional use of self/resources (e.g. non-labeling language, client confidentiality with other clients, personal disclosure as it relates to client goals).
  • Provide psycho-education as needed on developmental tasks and relationship challenges, including domestic violence/healthy relationships, drug use/abuse, gender roles, and self care/coping.
  • Utilize a Harm Reduction approach designed to non-judgmentally address client’s choices in terms of the function of their behavior in their environment.
  • Utilize Motivational Interviewing Techniques within Positive Youth Development framework.

Time requirements/training

  • Flexible hours to accommodate participant's schedules. Case manager must be able to work with a team of professionals to multi-task, prioritize, and provide crisis intervention as needed.

Education/experience requirements: BA/BS in Social Work, Psychology, Counseling or related fields strongly preferred. Two years experience with foster youth required without a related degree. Spanish speaking preferred.
