Community Health Representative / Health Promotion Adult Health

SUMMARY

This position is staffed within the Tribal Health Maintenance Program and reports to the Program Director. The Community Health Representative provides direct outreach, education, and support services to Cocopah Tribal members, promoting good health, preventing chronic disease, and reducing risk factors. The CHR serves as a bridge between the community and healthcare systems, supporting culturally appropriate care and improving access to essential health services.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Assist in developing and implementing public education materials, workshops, and outreach activities focused on chronic disease prevention and health promotion.
• Conduct one-on-one and group education sessions for individuals and families on topics such as nutrition, physical activity, diabetes management, medication adherence, foot care, and other chronic disease areas.
• Make home visits to educate clients, monitor health conditions, take vital signs, deliver medications, explain instructions, and provide support as needed.
• Refer patients to appropriate outside resources, specialty services, or higher levels of care when necessary.
• Assist families by providing health information, education, supplies, and support for chronic disease self-management.
• Maintain accurate documentation of patient care, home visits, daily activities, and program status.
• Record patient condition, treatment, and assistance provided in the chart within one (1) day of service.
• Prepare and submit monthly activity reports to the Program Director.
• Attend staff meetings, training workshops, and seminars as required by the Director and/or Administration.
• Assist with other program activities, community events, and health promotion campaigns as needed.
• Transport members to non-emergency medical appointments when required.
• Communicate and coordinate with primary care providers for patient advocacy, scheduling, and follow-up education.
• Attend weekly Case Management Meetings as assigned.
• Assist with grant program implementation, reporting, and development as assigned.
• Administer Naloxone in good faith to individuals suspected of experiencing an opioid overdose (training provided).
• Conduct wellness checks for high-risk clients, including elders, individuals with chronic conditions, and those recently discharged from hospitals.
• Support emergency response activities when directed (e.g., wellness checks during extreme heat, cold, or community emergencies).
• Promote culturally appropriate health practices and support integration of traditional healing when appropriate.
• Maintain confidentiality and follow HIPAA and Cocopah Tribal policies at all times.

KNOWLEDGE, SKILLS, AND ABILITIES

• CNA or Community Health Worker certification preferred.
• Ability to take vital signs and basic clinical measurements.
• Basic knowledge of nutrition, physical fitness, chronic disease management, and health education.
• Strong interpersonal skills and ability to build trust with Tribal members, elders, and families.
• Ability to work collaboratively with THMP staff, IHS service unit staff, Tribal departments, and outside agencies.
• Strong communication skills, both verbal and written.
• Ability to speak the Cocopah language is preferred.
• Ability to drive a car/van and assist clients with mobility needs.
• Familiarity with local and regional community health resources.
• Ability to maintain accurate records and documentation.
• Must complete the Indian Health Services Community Health Representative Basic Training Model within the first 120 days of hire.
• Cultural sensitivity and understanding of Tribal community health needs.
• Ability to work independently, manage time effectively, and respond to urgent needs.
• Basic knowledge of infection control practices and safe home‑visit procedures.

QUALIFICATIONS

• Valid Arizona or California Driver’s License.
• CPR, First Aid, and AED Certification (may be provided if other qualifications are met).
• Must be able to undergo and pass a DPS/FBI fingerprint background investigation, and cannot have two (2) or more misdemeanors or felony convictions.
• Testing may be required.

EDUCATION AND/OR EXPERIENCE

• Associate’s degree in a related field or completion of a trade school program preferred.
• Professional or technical certification in health, wellness, or community health preferred.
• Minimum of 2 years of work experience in a related field (healthcare, community outreach, social services, or wellness programs).
• Experience working with Native American communities preferred.
