Job SummaryProvides overall coordination for care management in the acute care continuum by establishing a comprehensive programwhich includes case management with utilization review, care coordination, discharge planning, and social work. Supervises and coordinates the delivery of all Case Management Services including acute care, rehabilitation and outpatientcase management services. This includes staff education, hiring, coaching, performance management and mentoring of staff.Responsible for financial management of department including review of productivity and staffing. Responsible foroperations of CMS department including coordination of UM committee and UM reports. Serves as a clinical resource personfor case management services for other hospital departments. Acts as a community liaison with external review agencies,home care providers, durable medical equipment providers, medical staff and other community resources used for dischargeplanning. Has oversight of Key Performance Indicators for Case Management and provides feedback and direction to CaseManagement team. Targets are met so that case management remains quality focused

What You Bring/Job Requirements

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:

Minimum Experience Requirements5 years experience in Nursing, 3 of which include Case Management in a hospital setting.Prior supervisory/management experience in Case Management.Experience in Acute Care setting, preferred. Including discharge planningand clinical utilization review.

Bachelor's degree

License/Certifications Requirement

Current Colorado RN License
