DISTINGUISHING FEATURES OF THE CLASS:

The Elderly Services Liaison assists community elders requiring social, educational, medical or financial services;

assists in identifying problems to determine services needed and refer elders to appropriate service agency. This

position provides personal and professional liaison services in concert with other services by the department.

ESSENTIAL FUNCTIONS:

 Interview individual elder to determine needs and provide appropriate services.

 Assist elders in filling out applications for other entities that provide services to the elderly.

 Arrange for delivery of emergency food boxes when necessary.

 Make appropriate referrals to other agencies and programs (to include other local, state, and/or federal) when

necessary.

 Receive referrals from programs and agencies for elderly who are in need of services.

 Provide liaison services between Social Security Administration, AHCCCS/ALTCS, BIA Realty, Tribal

Administration and other entities providing services to the elderly.

 Serve as liaison on behalf of the elder when referrals are made to community service agencies.

 Make home visits when necessary to assess needs of elderly; provide information and services when

appropriate.

 Arrange and schedule appointments with community resources and services in accordance with elder needs

assessment.

 Maintain current files of community resources and make information available on how families can obtain

such services.

 Create and update computer database of elder clients, record contacts, and maintain records required to follow

up.

 Provide in-service training to requesting programs/agencies that provide services to the elderly.

 Perform other related duties as assigned.
