A Family Support Coordinator acts as the primary contact for families in need of welfare, emotional guidance, or other support. Their job is to serve as a bridge between a family and any relevant support agencies. Their main goal is to reduce the time that families in trouble have to wait for care. Under direct supervision of the Program Manager, this role provides social service coordination to families in Cochise County with children age 0-5 years old, as well as information/referrals to support and assist parents with the tools to promote healthy child development in the early years of life. A Family Support Coordinator will provide comprehensive family services for a minimum of 90 days up to 120 days to support the families Protective Factor Framework and ensure the family thrives.




Previous experience and certification for the following assessment models are highly desirable, but not required:

  • Intimate Partner Violence Screening


1. Provide family support coordination services at the designated Family Support Coordination (FSC) hubs throughout the First Things First Cochise Regional Partnership service area.

2. Participate in regular monthly reflective supervision with Program Manager. Create an annual Professional Development plan.

3. Participate in ongoing Continuous Quality Improvement (CQI) efforts.

4. Conduct activities that support the agency's strategic goals.

5. Recruit and maintain a caseload of up to 20 families.

6. Provide services at regularly scheduled consistent intervals for 90-120 days depending on the level of assessed need. FSC will conduct weekly 1-hour check-ins with families, and at least 1 weekly check-in per month must occur in person.

7. Enter all services provided into the Visit Tracker Database system.

8. Conduct independent client-based risk assessments for assigned families at intake.

9. Develop goals, treatment plans, and processes for assigned families at intake.

10. Promote positive, strength-based interactions between staff, participants, potential participants, community service providers/members, and internal/external key partners to maintain retention regarding participation and collaboration.

11. Plan and facilitate community social events for participants in the program monthly.

12. Attend outreach events on a weekly basis.

13. Track progress for assigned families to determine level of program completion on a weekly basis.

14. Connect families to resources that help them reach their goals and address their needs on a weekly basis.

15. Provide educational evidenced-based printed materials to families in relation to the services, referrals, and goals in their service plan.

16. Responsible for understanding and complying with CPLC, DCS and any other funder/contract policies and procedures.

17. Responsible for successful completion of specified and program required trainings by the due dates.

18. Track participant's needs, activities, visits, and procedures that are required for program sustainability daily.

19. Prepare and submit timely documentation and reports, maintain client records in accordance with professional standards and funder requirements.

20. Assist with family discharge based on completed services.

21. Testify in court regarding cases according to Parenting Arizona policy when subpoenaed.

22. Responsible for reporting incidents and/or emergencies to their direct supervisor immediately.

23. Serve as mandated reporter of child abuse if disclosed in any form and must immediately report the incident to their supervisor and make a DCS Hotline report and implement service plan.

24. Attend staff meetings and staffing as requested.

25. Maintain healthy professional boundaries with staff, participants, potential participants, community service providers/members, and internal/external key partners to maintain effective quality services.

26. Display respect, dignity, and diversity to any participant, potential participant, child, funder, and internal/external key partners in a non-judgmental manner that will honor the participant's rights to privacy and confidentially.

27. Adhere to all contractual agreements between Parenting Arizona and funding sources as well as agency policies and procedures.

28. Other duties as assigned.


Bachelor's Degree (preferred) in child development, family studies, psychology, social work or related area of study, or experience equivalent of a Bachelor's degree including completion of a 2 year +AA degree plus 2 years of experience or

HSD/GED plus four (4) years of experience working directly with culturally & economically diverse populations of parents, infants, and children. Candidates who do not meet Bachelor's Degree criteria are subject to the approval of the funder and must agree to a professional development plan upon hire.
  • Must be at least 21 years of age.
  • Must have a valid driver's license.
  • Must have access to safe, reliable transportation with valid automobile insurance.
  • Must have a valid Arizona Level One Fingerprint Clearance card.
  • Complete CPR and First Aid certifications within 90 days of hire and maintain the certifications throughout employment.
  • Minimum of 20 hours per Fiscal Year of continuing education and competency building, including but not limited, to Relias Learning, Conference, Workshops, and/or Outside Training.
  • Knowledge and/or experience with infant & child development, parent-child attachment, possible behavioral health dynamics of caregivers, as well as the dynamics of child abuse & neglect.
  • Knowledge and/or experience with strength-based and family-centered provision of primary prevention services.
  • Able to identify individual/family strengths and planning/advocating around individual/family goals.
  • Knowledge and/or experience with community resources as well as how to navigate/access these resources for families (some examples of services include Social Service Agencies, Provider Systems such as behavioral health, and division of developmental disabilities.)
  • Possess strong communication and interpersonal skills (e.g., nonjudgmental, objective, reflective, empathic, patient, tactful).
  • Possess strong written and verbal communication skills to promote clear understanding.
  • Be self-motivated and work independently, cohesively within team dynamics, and under direct supervision to effectively meet program requirements/objectives.
  • Ability to plan, organize, schedule, keep appropriate records, and develop effective programmatic communications with supervisory guidance.
  • Knowledge and/or experience in cultural diversity, substance abuse, sexual abuse, family violence that may have an impact of child abuse and neglect on child development, required.
  • Demonstrate interest and ability to advocate with and on behalf of low-income, diverse families, required.
  • Previous experience in family support coordination or resource navigation services, preferred.
  • Bilingual (English and Spanish) highly desirable (will consider other languages)

Effective January 3, 2022, all CPLC Employees and all Service Providers, are required to be Fully Vaccinated against COVID-19. Newly hired employees, who are not Fully Vaccinated at the time of hire, are expected to begin a vaccination series immediately upon hire. CPLC recognizes medical and religious exemptions to the COVID-19 vaccination requirement, in accordance with applicable law and upon approval by CPLC. However, any person approved for an exemption, without exception, is subject to masking, weekly COVID-19 testing at their expense, and other requirements established by CPLC from time to time.


Position requirements: This position is both a mobile and a sedentary position with periods of light physical activity and is performed mostly in office and residential settings based on the areas assigned. Positions typical in nature require regular walking or standing; sitting, lifting and carrying up to 20 pounds; climbing stairs, bending, reaching, holding, grasping and turning objects; and using fingers to operate computer keyboards. This position will require the ability to speak normally and to use normal or aided vision and hearing. Position works a full-time, shift-based schedule determined by family and funder needs. Must anticipate frequent schedule changes and be available to work mornings, days, evenings and weekends as needed because home visits occur based on the family's availability and times that are convenient for them to meet.

Travel: Significant local and interstate travel will be required, including but not limited to, client residences and community locations, as well as to Chicanos Por La Causa, Inc., locations, its affiliates and/or subsidiaries. Some interstate and intrastate travel for training purposes may be necessary. Technology will be utilized to minimize travel whenever feasible. The travel requirements for this position are anticipated to be approximately 30 percent, mostly local.

Continuous Learning: PE is required to participate in ongoing continuous learning and professional development at a minimum of 20 hours per year
