We are now offering a sign-on bonus of $1500 upon hire. Eligible for $1,000 after your first 60 days and $500 eligible to be received after 6months.

Job Preview at a Glance:

The Intake Specialist is a specialty position working with adult individuals who have been released and are transitioning from incarceration. Position works as part of a multi-disciplinary team with the Pathways Program and the County Jail System to successfully transition persons from jail services to community stability. Requires interface with Correctional and legal system staff, individual client contact and service provision, case management, Community Outreach, service planning and coordination, crisis and behavioral health interventions/services, referral, and follow up. Position works as a partner to those served to ensure success and supports team processes.

Location & Schedule:

This a Monday -Friday schedule 8am-5pm working out of the Pathways Program adjacent to the jail and out of the Flagstaff Outpatient Clinic.

Pay: Pay $19.00 hourly

Responsibilities:

  • Completes intake/enrollment for individuals released from incarceration
  • Facilitates connection up to and including reengagement into outpatient services and behavioral health/psychiatric medication appointments as appropriate, connection to MAT induction and continuation as appropriate
  • May provide crisis triage support as needed and when appropriate
  • Facilitate linkages to care to support clients in accessing services, including completion of warm handoff to community partners in partnership with Pathways staff as appropriate for behavioral health care coordination, regardless of health plan enrollment (AHHHCS/Medicaid/Private). For example, securing bed dates
  • Works in cooperation as part of a multi-disciplinary team and collaborate with Pathways staff
  • Will be located inside the Pathways Program at the County Jail
  • Develops working rapport with team, persons served, and legal system to effectively perform smooth and seamless transition.
  • Responsible for case management and carries an independent case load.
  • Follows up on all referrals in a timely manner.
  • Provide screenings & and ongoing assessments.
  • Provides crisis intervention, de-escalation, and Behavioral Health interventions.

  • Acts as a responsible guest of the jail assessing client situations, recommending services, coordinates care, and developing rapport with persons served as to set the framework for community work to follow
  • Provides outreach work based on the needs of the team and individuals served.
  • Works to reduce recidivism and meet outcome goals set for program performance.
  • Familiar with AHCCCS and MMIC eligibility, rules, and entitlements
  • May work with persons to reestablish benefits
  • Reviews with client the expectations upon release and follows up as needed.
  • Plan, coordinate, and assist client with immediate and basics needs for release.
  • Make referrals to appropriate services in cooperation with clinical teams, and monitor client participation & compliance.
  • Works to provide and or facilitate the prescribed treatment planned accordingly
  • Develops and implements a tracking system and follow-up process for referrals in a timely manner.
  • Document activities for tracking and reporting according to documentation standards
  • Responsible for contractual obligations and fulfillment
  • May be required to work flexible hours as needed to best serve the program and client needs which may include weekends.
  • Required to carry a company smart phone
  • Required to drive personal or company vehicle and keep appropriate documentation

Required Education and Experience:

  • High School Diploma or GED plus some college course work in Behavioral Health or related fields, plus one year directly related job experience, or the equivalent combination of experience and education
  • Required to maintain a valid Arizona drivers license and pass motor vehicle screening for insurance purposes in order to transport clients, based upon need/requirements of the clinic and/or program.
  • Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.

  • Peer Certified preferred.
