We are seeking a Integrated High Needs Coordinator to join our Buckeye Outpatient Clinic. This location provides mental health treatments, including medication monitoring and assistance for persons with addictions.
The Integrated High Needs Coordinator will work in-office Monday-Friday, standard business hours (no evenings, weekends or holidays).

The Integrated High Needs Coordinator serves as a central support within the interdisciplinary team. This staff member provides clinical support by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions and resources.
- Assists youth and families in securing services
- Provides education to members regarding treatment planning, coordination of care and the development of informal and community supports
- Facilitates CFT meetings, staffing using information gained from the comprehensive assessment and ongoing treatment
- Develops, implements, and revises a relevant safety/support plan for the family
- Provides accurate documentation of contact with members
- Minimum of 21-year of age
- Bachelors degree in the behavioral health field plus one year of relevant experience in behavioral health
- Valid AZ drivers license, and Motor Vehicle clearance
- Ability to obtain fingerprint clearance card
