Hope Home Care is looking for a reliable and professional Physical Therapist to treat patients in Miamisburg, Dayton, and surrounding areas. Qualified candidates will be dedicated to performing work at a high level of quality not only with direct patient care, but in the documentation completed for that care as well. Must be available to answer phone calls/messages while in the field or return calls/messages in a timely manner.

JOB DESCRIPTION SUMMARY

The physical therapist contracted or employed through Hope Home Care, LLC is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

POSITION QUALIFICATIONS

1. Possesses a degree from a baccalaureate, master's, or doctorate program in physical therapy approved by an accredited organization.

2. Licensed to practice as a physical therapist within the state.

3. Prefer 2+ years of home health care experience as a physical therapist.

4. Demonstrates good verbal and written communication and organization skills.

5. Possesses and maintains current CPR certification.
