Position Summary:
Responsible for planning, implementing, and evaluating physical therapy services for patients in concert with the total care plan. Provide quality care to attain patient’s optimal functional level.

Managerial/Supervisory Responsibilities:
None

Accountabilities/Essential Job Functions:

  1. Assess the patient’s physical therapy needs:
    1. Obtain a medical history, particularly as it is related to the present condition, from the patient, family, physician, team members and/or previous records.
    2. Evaluate the patient in the following areas: ROM, muscle status, balance skills, coordination, functional activities, gait analysis, postural alignment, endurance, learning abilities and mental status.
    3. Assess the facility environment to determine and make adaptations as needed for the ease and safety of care and the maximal independence of the patient.
    4. Assess equipment needs in the facility and assist in obtaining same.
    5. Evaluate the patient’s condition, family and home situation to determine what assistance will be available from family members in caring for the patient; determine if additional services or resources are needed. Assess equipment needs and medications for safety and progressive recovery at LLOC.
    6. Instruct caregivers to special needs of the patient, including but not exclusive to positioning, transfers, and mobility function.
      1. Develop an individualized therapy plan:
        1. Utilize the assessment data to develop a care plan based on the problem-oriented record format: develop a written treatment plan, including objectives and methods of teaching and adjust information to patient’s current level of understanding: prepare and instruct PT approaches; adjust patient care plan as appropriate.
        2. Interpret hospital and clinical services as the patient and family are involved in the development of a care plan.
        3. Consult with appropriate disciplines and request their involvement in care, using their information and ideas to develop the plan.
        4. Consult with physician and obtain consent to proceed with treatment plan.
          1. Assure implementation of the therapy plan:
            1. Provide physical therapy services and treatment consistent with POC, requiring specialized skill and ordered by the physician.
            2. Provide physical therapy procedures and techniques which may include: muscle re-education, progressive muscle strengthening exercises, ROM, gait training, functional training, application of physical modalities, posture, breathing exercises, coordination and balance exercises. Modify treatment plan as indicated by patient condition progress.
            3. Instruct caregivers and family for maximal function and safety in the home environment, including proper use of equipment, body mechanics and guarding techniques.
            4. Instruct, supervise and counsel caregivers and family members in continuity of care management.
            5. Make referrals to other disciplines as appropriate.
            6. Provide written feedback to referral source as appropriate
            7. Maintain appropriate and timely documentation to patient progress.
              1. Administer planned an informal learning experiences for the patient, delegating appropriate reinforcement to other team members.
                1. Maintain responsibility for the care given by the Physical Therapy Assistants/PT Aide
                2. Assume or share responsibility with other team members for care given by Rehab Aide.
                3. Communicate patient care and progress in case conferences
                4. Chart therapy services rendered using the SOAP note.
                5. Record units according to rehab department procedure.
                  1. Evaluate the therapy plan:
                    1. Review the written plan and its effect on patient care and make appropriate modifications at least every 30 days.
                    2. Obtain feedback from the other team members regarding the plan’s effectiveness.
                    3. Explore with the staff, patient and /or family, progress in care.
                    4. Consult with the attending physician concerning alteration of the therapy plan.
                    5. Review patient’s record in appropriate utilization review setting
                    6. Meet as needed with the Rehabilitation Manager regarding patient care problems and how these may be resolved.
                      1. Perform other duties as assigned.
                        1. Participates in CE programs to insure continued professional development; shares knowledge and technical skills with other department members.
                        2. Participates in department program development projects as directed
                        3. Attends and participates in professional staff meetings and in-services.
                        4. Performs other duties as required

                        5. Qualifications

                          Education/Training: Master’s or bachelor’s degree or a graduate of an accredited physical therapy school as approved by the American Physical Therapy Association in collaboration with the Council on Medical Education or the American Medical Association.

                          Experience:

                          License/Certification: Current license to practice physical therapy in Washington State.

                          Skills: Strong written, verbal and interpersonal skills.

                          Environmental and Job Hazards:

                          Physical and Mental Demands: Ability to safely lift 25 lbs.
