As an ACT Team Specialist on our Assertive Community Treatment (ACT) team, you will play a critical role in helping individuals with severe mental illness achieve stability and thrive in their communities.
Under the direction of the ACT Clinical Coordinator, you will work directly with a multi-disciplinary team to deliver comprehensive case management, rehabilitation, and crisis intervention services. Your work will directly impact the lives of those you serve, helping them achieve their goals in the community, at home, and at their place of employment. Youll ensure that each individual receives tailored services that foster recovery, independence, and a sense of empowerment.
What Youll Do:
- Provide individualized case management, rehabilitation, support, and crisis intervention services to ACT team members, meeting them where they are, physically and emotionally
- Work in a multi-disciplinary team to develop and implement personalized individual service plans (ISPs) to meet each members unique needs and goals
- Coordinate or provide direct services in the community, including at patient homes, workplaces, and other community settings
- Facilitate access to vital resources, ensuring members receive the support they need to achieve independence and lead fulfilling lives
- Foster trusting relationships with ACT team members, offering encouragement and empowerment during times of crisis, transition, or recovery
- Document and track progress, ensuring comprehensive service delivery and the continued success of each individual in the program
- Actively contribute to team meetings and collaboration, bringing your insights and expertise to the table as part of a dynamic, supportive care team

Hourly Pay Rate: $19.73 - $29.10
Qualifications
Education:
- Bachelor's degree in a field related to Behavioral Health or
- Any other bachelors degree and one (1) year of full-time behavioral health work experience or
- Any associate degree and two (2) years of full-time behavioral health work experience or
- Eighteen (18) credit hours of post-high school education in a field related to behavioral health and two (2) years of full-time behavioral health work experience or
- A high school diploma or GED and four (4) years of full-time behavioral health work experience.
Experience:
- Requires a minimum of one (1) year of experience working with adults with a serious mental illness, demonstrating an understanding of the required knowledge, skills, and abilities.
- Previous experience working with an ACT team.
Certification/Licensure:
- Requires possession of and ability to retain a current valid Arizona-issued driver's license in good standing and must carry the AZ legally always required minimum insurance coverage on a personal vehicle.
- Pursuant to Arizona Administrative Code R9-10-306 CHAPTER 10 of the Dept. of Health Services, personnel must be at least 21 years of age or at least 18 years of age and licensed or certified under A.R.S. Title 32 and providing services within the personnel members scope of practice.
Knowledge, Skills, and Abilities:
- Must understand the ACT model, trauma-informed care, and recovery-oriented service delivery.
- Requires home and community visits, knowledge of service needs, entitlement, and community resources.
- Requires the ability to work a flexible schedule that includes weekends, evenings, and on-call rotation.
- Requires the ability to have a reliable form of transportation to travel throughout the community with transportation of ACT members as needed.
- Must have good verbal and interpersonal skills to interact with clients, visitors, and coworkers.
- Requires the ability to maintain highly confidential information, including a working knowledge of HIPAA.
- Must have the ability to work independently and seek supervision appropriately.
- Requires the ability to read, write, and speak effectively in English.
