Job Description
Telephonic Case Manager responsible for engaging membership and addressing member needs related to clinical conditions, gaps in care closure, navigation within complex health systems.

Work schedule may include 8 or 10 hour shifts monthly rotational. No weekends.
Fully remote.

Required Qualifications
5 years clinical experience in management of individuals with chronic and acute medical conditions
Clinical experience in oncology, diabetes, immunological diseases (RA, HIV), maternal/child
RN Licensed in the state of residence active and unrestricted
Holds a compact nursing license in the state of California

Preferred Qualifications
Telephonic disease or case management experience
Strongly preferred Bilingual Spanish, Mandarin, Tagalog, Haitian, Creole, Korean
